PRE-APPOINTMENT INTAKE FORMS

• I am legally required to collect tax and have included it in the total price listed in my treatment guide. You will see a breakdown on the booking tab when you reserve your treatment time.
• Services operate by appointment only.
• Your appointment is important to me and reserved especially for you. • Should you need to cancel or reschedule I respectfully request at least 24 hours notice. Failure to provide this notice will result in being asked to pay 50% of the next service upfront when rebooking.
• Please understand that when appointments are missed or cancelled without giving enough notice, the opportunity to fill that appointment time is missed as well as the opportunity for other clients wanting to receive services.
• The online booking system sends out 2 reminders in advance via text message and email.
• Please note that a late arrival may result in needing to customize your service to fit the remaining time AND the full service price will apply.
In the event of a no-show, the full cost of all upcoming appointments will be due in advance.
